Nursing Care Plan and Specimens Essay Quality is a broad term that encompasses various aspects of nursing care (Montolvo, 2007). The National Database of Nursing Quality Indicators [NDNQI] is the only national nursing database that provides quarterly and annual reporting of structure, process, and outcome indicators to evaluate nursing care at the unit level (Montolvo, 2007). Nursings foundational principles and guidelines identify that as a profession, nursing has a responsibility to measure, evaluate, and improve practice (Montolvo, 2007). The purpose of this paper is to analyze the ââ¬Ëmislabeled specimenââ¬â¢ indicator for an inpatient rehab unit and devise an action plan based on best practices to decrease the incidence of mislabeled specimens. Analysis of the data According to Dock, (2005) accurate specimen identification is a challenge in all hospitals and medical facilities. Ensuring that specimens are correctly identified at the point of collection is essential for accurate diagnostic information (Dock, 2005). A mislabeled specimen can lead to devastating consequences for a patient (Dock, 2005). Specimen misidentification can be serious, resulting in misdiagnosis and mistreatment (Dock, 2005). For the second quarter of FY09, the rehab unit met their target of zero mislabeled specimens. The third quarter yielded two actual mislabeled specimens with a variance of two. The fourth quarter actual was one with a variance of one. The first quarter FY10 showed an actual of one and variance of one. For the FY09 the rehab had a total of four mislabeled specimens. This indicator was chosen because of the magnitude of this medical error. Nurses, administrators and laboratory personnel must collaborate and create ways to decrease the mislabeling of spec imens. Nursing plan Nursing interventions to decrease the number of mislabeled specimens and improve actual indicator scores are: 1) Ensuring proper identification of patient 2) The use of electronic technology and 3) Bedside labeling. Each of these interventions will positively impact patient outcomes and reduce errors. According to The Joint Commission [TJC], proper patient identification is best practice for decreasing mislabeled specimens (The Joint Commission as cited by Sims, 2010). National Patient Safety Goal [NPSG], 01.01.01 states that healthcare providers should use at least two identifiers to identify patients. For example, the patientââ¬â¢s full name and date of birth is used to properly identify a patient (The Joint Commission, 2014). The strongest intervention to reduce labeling errors is the addition of barcode technology (Brown, Smith Sherfy, 2011). The use of automated patient identification and specimen collection techniques can be an additional safety net for routines that are vulnerable to error, especially when coupled with strong systems designs (Brown et al., 2011). Brown et al (2011), found that the clinical applications of electronic and information technology support can assist in the identification, control, and reduction of error rates throughout the process. According to the World Health Organization [WHO] healthcare providers should encourage the labeling containers used for blood and other specimens in the presence of the patient (World Health Organization, 2007). This would suggest labeling specimens at the patientââ¬â¢s bedside or before leaving the room. Nurses should never label specimens before collection as this could lead to serious errors. Summary In summary, NDNQI indicators serve as a schoolmaster that holds nursing accountable to practice. An analysis of ââ¬Ëmislabeled specimensââ¬â¢, found that an inpatient unit had a total of four incidents for FY09. While this does not seem like a great deal of errors, any one error could have detrimental consequences. The plan of action based on best practices is to properly identify the patient, using electronic technology, and labeling specimens at the bedside. Analysing The Religious Views On Euthanasia Philosophy Essay When it comes to religious point of views on issues, euthanasia is one of the most controversial moral topics of all time. Euthanasia comes from the Greek word ÃŽà µÃ ¡Ã ½Ã ÃŽà ¸ÃŽà ±ÃŽà ½ÃŽà ±Ã ÃâÃŽà ¯ÃŽà ± meaning good death. Euthanasia can be defined as the practice of ending a life in a technique that reduces pain and suffering ( Eike-Henner 11). Religions such as the Roman Catholics and Orthodox Judaism oppose euthanasia on the grounds that it invades Gods territory of life and death. Life is considered a gift from God and should only be taken back by God. There are different forms of euthanasia: voluntary/involuntary and active/passive. Active euthanasia is the inducement of death by taking specific steps such overdose of painkillers or sleeping pills. Passive euthanasia, on the other hand, is the discontinuation of medical treatment in a terminal case so as to allow the patient have a natural death. Voluntary euthanasia is killing a person for the purpose of relieving pain from terminal or incurable illness or injury with their consent. Finally, involuntary euthanasia is killing a person for the purpose of relieving suffering without first obtaining the persons consent. This paper will examine the ethical views of the Roman Catholics and the Orthodox Jews on the act of euthanasia. The Roman Catholic teachings have a precise definition for euthanasia. Pope John Paul II describes it as an act or omission which of itself or by intention causes death, with the purpose of eliminating all suffering (Evangelium vitae [Ev], 1995, n. 65). The Church teaches that it is morally unacceptable and constitutes it as a murderous act, which is contrary to what they call the dignity of a person. Namely that every individual is born with dignity, and they are an estimable value. They value the human life, because a person indeed is priceless, for they are created by God. Euthanasia is an act that denies respect of God, the creator. Even if a person is acting on good faith, nothing changes the nature of what euthanasia is; at the very base euthanasia is an act of murder. When it comes to physicians assisted suicide the Roman Catholics describe it as a grave sin against God, because it suggests that a person is in charge of their body instead of God the creator. Physicians assiste d suicide can be defined as a doctor or medical staff person prescribes a lethal amount of medication with the intent of assisting a person commit suicide (John 151). The patient then takes the dose or turns the switch. Since preserving life is the greatest good for Catholics, they believe that the 5th commandment in the Old Testament of the Bible says it all Thou shall not kill (John 14). They believe this applies to killing oneself or killing someone else who has asked to die such as a terminally ill patient. No human being has the authority to make life and death decisions. Killing denies the victim something that he or she values greatly, continued life or a future. Moreover, since continued life is necessary for pursuing future endeavors, killing brings the suppression of all of these plans. Therefore, Catholics believe that doctor assisted-suicide deprives an individual of a future that they potentially couldve had. In the case of sustaining life, it is said that one is obligated to use ordinary means, but one is not always obligated to use extraordinary means. The term ordinary means refers to all medicines, treatments, and operations, which offer a reasonable hope of benefit for the patient and which can be obtained and used without excessive expense, pain, or other inconvenience. By contrast, extraordinary means refers toall medicines, treatments, and operations which cannot be obtained or used without excessive expense, pain or other inconvenience, or which, if used, would not offer a reasonable hope of benefit. (John 30). These two concepts are derived from the general distinctions between avoiding evil and doing good. Thus, the duty to preserve life is a duty to do good which has reasonable limits, whereas euthanasia or mercy killing is evil and hence must be avoided. The Catholic position is a firm no to active euthanasia, but passive euthanasia could be acceptable in certain limited case s. For example, a therapy which was only prolonging death in a hopeless case and is morally acceptable, since the ultimate purpose is not suicide or euthanasia, but rather, an act whose moral object may be precisely described as allowing the patient to die for legitimate reasons. According to Roman Catholics, everything happens for a reason, by trying to control life or death, we would be interfering with Gods will. The sanctity of life principle, which rests on the human persons unique relationship with God, is the basis of the Churchs honoring of human life as a basic value. Under this principle, direct intervention to end the life of a patient in a terminal condition would not be condoned. This negative position also follows from the religious principle of divine sovereignty, the idea that God has the right over life and death, and the end of human life is not subject to a persons free judgment (Eike-Henner 32). Catholics believe that the end of life is a very spiritual time, and euthanasia happens to interrupt this spiritual moment. As stated in the bible So God made man to his own image, to the image of God he created him: male and female he created them. (Genesis 1:27). (Eike-Henner 37). Saying that God made man to his own image, does not mean that human beings look like God, but rather human beings have distinct capacity for rational existence that permits them to see what is good and want what is good. As people flourish these capabilities, they live a life that is equivalent to Gods life of love. The Roman Catholic Church affirms mans basic unity and his living in this world for God and for others. More than simply living in this world, the Church affirms life as a participation in the life of God in such a way that earthly life makes it possible and prepares one for participation in eternal life. Everyone faces death in its inevitability, a reality occurring only once. Catholics however, believe that death, like birth and life, is part of the same hu man condition. One is born in order to live and eventually die, having made the very best one could of all three conditions in one continuing totality. Death comes at the end of life so that one may go forward and meet in fullness having completed that life. Judaism is, in its essence, a life affirming faith. And Thou Shall live by them Gods Commandments, (Leviticus 18:5) is the central imperative of Torah. Orthodox Judaism is one of the modern Jewish movements who believe that both the Written and Oral Torah are divine and represents the word of God. (John 37) When it comes to the topic of euthanasia, Orthodox Jews believe that man is charged with preserving, dignifying and hallowing his life. But he is not given the absolute title over his life and body, only its caretaker, for life is a gift from God, to whom the individual is responsible for its preservation. My God, the soul with which Thou hast endowed me is pure. Thou hast created it. Thou hast formed it. Thou has breathed it into meà ¢Ã¢â ¬Ã ¦So long as there is soul within me, I give thanks before Thee, Lord my God. (John 99) Man is never called upon to determine whether life is worth living. According to Orthodox Jews life is a gift of infinite value. Since infinity is, by definition indivisible, it clearly makes no moral difference whether one shortens life by many years or by a few minutes, or he was old and physically or mentally disabled. In such cases, suicide is no less culpable than murder. When it comes to physician assisted suicide, one aspect needs to be taken into consideration, The Hippocratic Oath. The physician derives his ethical warrant from the Hippocratic Oath, which on one hand, requires that he desist from supplying deadly medicine to his patients for the purpose of hastening their demise, or even from offering advice which may lead to suicide, and on the other, that he relieves suffering. The discontinuation of artificial life supporting therapy is determined by the halakhic status (Jewish Law) of the terminally ill patient. A person who is regarded as having no chance of survival beyond 12 months, in spite of the application of all unknown therapeutic techniques is called a treifah (Eike-Henner 87). Therefore, regarded as a living being, and its ephemeral life has full halakhic value. However, if one kills a treifah is guilty of murder. However, if death is imminent, in consequence of loss of a vital organ or massive arterial bleeding it is termed a nevei la me-hayyim (Eike-Henner 87) and is not considered as already dead, because recovery is possible in exceptional cases. The final phase for terminally ill patients is called goses, (Eike-Henner 88) which is the inability to swallow own saliva. According to Judaism it is generally held that most patients in this condition cannot recover and that death will come within 3 days. If thats the case, then, the physician may discontinue medication for the existing condition and does not need to initiate treatment of an unrelated and possibly fatal complication. In all cases, nutriments as well as painkillers should be supplied. Among rabbinic ethicists, Rav Moshe Feinstein holds that when a patient is gripped by unbearable pain and suffering, nature should be allowed to take its course. Thus when a patient is on a respirator and the machine is temporarily removed for servicing, if the patient shows no sign of life the machine need not be restored (John 87). However, Orthodox Jews do allow f or the termination of artificial means of the patient if such means does not enhance the patients state and are the only things keeping the patient alive. It is also acceptable to take strong painkillers, even it they hasten death, as long as the drug is not the cause of death and the intention is to alleviate pain and not to cause death. Orthodox Jews do recognize the pain and suffering a patient experiences and the effect it has on the family, however, the Jewish faith has always responded against the termination of a persons life, even in the most severe cases when it may become evident to them that life is meaningless. Roman Catholics and Orthodox Jews have similar ethical views when it comes to the termination of ones life. They believe that suffering is used to purify ones soul, and God has the only right over a persons body. Dr. Kavorkian also known as Dr. Death is an American pathologist that performed assisted suicide in 1990s. From an Orthodox Jews perspective, Dr. Kavorkian is known as a murderer, because under the Jewish law, any form of active euthanasia is strictly prohibited and condemned as plain murder. Murder is one of the 3 fundamental sins prohibited by the Torah, and anyone who kills a person is guilty to the death penalty as a common murder. Similarly, the Roman Catholics disapprove physician-assisted suicide or euthanasia, on the grounds that it invades Gods property and to assist some in committing suicide is to commit murder, which goes against Gods unequivocal commandment Thou Shall not Kill. Euthanasia is one of the most controversial topics facing religions. The Bible and the Torah both state that human life is the property of God, and as humans we are only caretakers of that property, and have no consent of destroying something that does not belong to us. Every human life is valuable, regardless of their age, sex, or race. Catholics and Orthodox Jews would state that anybody who insists that they have the choice to terminate their life is denying the truth of their intrinsic relationship with God. People should not ask for euthanasia because no one knows the significance of each human being, for every human life is priceless

John Steinbeck's East of Eden: Modern Biblical Story of Cain and Abel " And Cain talked with Abel his brother: and it came to pass, when they were in the field, that Cain rose up against Abel his brother and slew him. And the Lord said unto Cain, ' Where is Abel thy brother?' And he said, ' I know not. Am I my brother's keeper?' And he said, ' What hast thou done? The voice of thy brother's blood crieth unto me from the ground. And now thou art cursed from the earth, which hath opened her mouth to receive thy brother's blood from thy hand. When thou tillest the ground it shall not henceforth yield unto thee her strength; a fugitive and vagabond shalt thou be in the earth.' And Cain said unto the Lord, ' My punishment is greater than I can bear. Behold, thou hast driven me out this day from the face of the earth, and from thy face shall I be hid. And I shall be a fugitive and a vagabond in the earth" (Genesis 4:8-1, KJV). The story of Cain and Abel is probably one of the most violent and unsettling stories of the Bible. It is probably the most unsettling because it tells the truth about mankind. Rejection is the one thing that all of mankind fears, and when one is rejected anger follows. With anger comes the need to commit a crime of revenge, and with this crime comes guilt. Almost everyone has experienced this in some way. East of Eden (1952) by John Steinbeck is a modern retelling of the biblical tragedy of Cain and Abel. The story is reenacted through the lives of two generations of the Trask family. Cyrus Trask, the patriarch of the family and his two sons Adam and Charles live on a farm in Conneticut. Charles cries out in anguish for his father's love, but his father ignores his cries and loves Adam best... ...il and do goodness. John Steinbeck writes, " We have only one story. All novels, all poetry, are built on the never-ending contest in ourselves of good and evil. And it occurs to me that evil must constantly respawn, while good, is immortal. Vice has always a new fresh young face, while virtue is venerable as nothing else in the world is" (415). In East of Eden, John Steinbeck creates this world of good and evil. All of the characters are faced with good and evil, and only one is able to triumph over evil. Cal is the representation of what everyone can be. Analysis of The Celestine Prophecy by James Redfield The Celestine Prophecy by James Redfield tells the story of a man who tries to learn and understand the nine key insights into life itself in an ancient manuscript that has been discovered in Peru. It predicts a massive spiritual transformation of society in the late twentieth century. We will finally grasp the secrets of the universe, the mysteries of existence, and the meaning of life. The real meaning and purpose of life will not be found in religion or in material wealth, but rather in things like auras. He comes across the insights in numerical order since that is how one must understand them. Overcoming an initial skepticism, the narrator's understanding grows with each insight. They include the following: 1. A Critical Mass. 2. The Longer Now. 3. A Matter of Energy. 4. The Struggle for Power. 5. The Message of the Mystics. 6. Clearing the Past. 7. Engaging the Flow. 8. The Interpersonal Ethic. 9. The Emerging Culture. In the first 15 pages, while the plot is still forming, at least eight basic ideas are introduced: a spiritual awakening is occurring in the world (p.4); humanity is evolving into a higher spiritual consciousness (p.4); seek the experiential (p.5); coincidences have spiritual significance (synchronicity) (p.6); the knowledge contained in the manuscript's insights has been hidden from most of the world (esoteric, secret knowledge) (p.8); anti-Christian attitudes (p.9); discover truth through experience (p.10); and when the student is ready, the teacher appears (p.15). These ideas are not always expressed in so many words, but their principles are. For example, the basis of the story is that the spiritual insights humanity needs are hidden in an ancient document, and must be uncovered if mankind is to advance spiritually. Not everyone, according to the story, is ready for or able to comprehend these teachings. The insights are for those spiritually ripe, the spiritual elite. The book i mplies that in time others will accept these ideas but for now the more advanced must lead until a critical mass of people have grasped the insights. The first two insights are that coincidences have a deep significance and that this decision-making should guide an individual. Later insights build on this, teaching the narrator that he should be guided by daydreams, intuitions and thoughts that ma... ... over several years time. Crossroads encountered several times in one day in The Celestine Prophecy probably happen over a much longer period to someone finding one's self. I enjoyed and was intrigued by the control dramas analogy that was presented in The Celestine Prophecy. They sure seemed to enhance understanding on the issue. Also this spiritual enlightenment story portrayed to me that healing to an awareness of our inner light can move right along when we are not encumbered by blame and guilt. There was no judgment implied as to which choices that one makes, it seemed to be simply a matter of preference if one chose to liberate or not. Acceptance of whatever is flows through the entire book. Naturally we would come to choose healing, and naturally we would tolerate those not ready to do so yet. But what blew me away is how blatantly and clearly the writing conveyed how the fearful phenomenon of dysfunctional codependency plays itself out--people attempting to live on each o ther's energy in lieu of tapping their own inner power. This can be seen in the scene where they seen the energies exchange between some people. All and all I enjoyed reading this novel by James Redfield. Fraudulent revenues will create misstatements that could have a material effect on the decisions of financial statement users. In 2002, WorldCom a telecommunication company, filed for bankruptcy. It was later revealed that the company was involved with improper accounting in two major forms. First WorldCom inflated revenues to increase profits, thereby increasing stock prices, and increasing the satisfaction of stakeholders. Second, the company understated line costs. Revenue is important to users of financial statements because it helps them evaluate a companyââ¬â¢s performance and prospects. WorldCom violated the revenue recognition principle by creating an account that did not come from the operating activities of the companyââ¬â¢s sales channel. WorldCom named this fictitious schedule corporate unallocated account. This action was unethical and illegal, and gave the company a very bad reputation. According to paragraph 25 of PCAOB Auditing Standard No. 5, because of its importance to effective internal control over financial reporting, the auditor must evaluate the control environment at the company. As part of evaluating the control environment, the auditor should assess the following, â⬠¢Does managementââ¬â¢s philosophy and operating style promote effective internal control over financial reporting? Has the company developed sound integrity and ethical values, and more importantly, do all employees understand these values, particularly top management? â⬠¢Does the Board or audit committee understands and exercises oversight responsibility over financial reporting and internal control? The control environment is what sets the tone for an organization and is the foundation for all other components of internal control. It provides discipline and structure and reflects the ethical values, integrity and competencies of the organization. The control environment is very important to effective internal control over financial reporting to an audit client like WorldCom, because good designs can prevent and detect frauds and errors. But because WorldCom had such a poor control environment, the company would require more testing for an audit. This shows that the board did not exercise oversight responsibilities over financial reporting or internal controls. According to PCAOB Auditing Standard No. 5 paragraphs 26 & 27, the top-side adjusting journal entries are when the executives record the entries, or when the accountants are asked by the executives to record the entries. A valid use of top-side journal entries is to allocate income or expenses from a parent company to its subsidiaries. However, top-side adjustments can also be used to improperly reduce liability accounts and increase revenue or decrease expenses. Companies undergoing mergers, acquisitions or restructuring are particularly susceptible to the fraudulent misuse of top-side journal entries. Necessary evidence to obtain include sales invoices, credit memos, customer master file list, analytical procedures, and accounting systems. In the auditing of WorldCom, we would require adjusting journal entries, the MonRev spreadsheets detailing revenue, the corporate unallocated schedule, the automated process for closing and consolidating operational revenue numbers, and propriety of a top-side journal entry made to their revenue account. We would also need the authorization of the CFO or any officers. Good ethics is very significant for an organization.
